APPLICATION AND AGREEMENT FOR PARISH SUBSIDIES 

I/We, ____________________________, the undersigned hereby submit this request to receive Tuition Subsidy from St. Joseph’s Parish, St. Patrick’s Parish or from St. Pius’ Parish for my
child(ren)’s enrollment at St. Brigid School for the 2009-2010 year.

Guidelines and Expectations 

I/We, _____________________________, hereby acknowledge and agree to the following guidelines and expectations in order to receive a tuition subsidy. 

1. The family must be a registered parishioners of the aforementioned parishes by September 1, 2008, in order to qualify for the parish subsidy for the school year beginning in September 2009.
   

2. The newly registered family will be expected to demonstrate a record of faithful participation in the life of their Parish.  In the event the pastor concludes that the family has not fulfilled its obligations to the Parish as a participating member in the life of the Parish, then he has the discretion to review the continuation of the tuition subsidy for the current year and for the subsequent year.  If tuition subsidy is terminated, the unpaid subsidy balance becomes the responsibility of the family and will be billed accordingly by St. Brigid School.
   

3. It is requested the family participate in the life of the Parish.“Participation” includes, for example, regular attendance at Sunday Mass, involvement in school and parish activities, as well as weekly financial contributions, as appropriate, based upon individual circumstances.
   

4. A newly registered family must provide verification that it was a registered, contributing and active member of its previous parish, as applicable.
   

5. If there is an outstanding tuition balance after June 1, 2009, this agreement for parish subsidy is null and void.
