SCHOOL TUITION CONTRACT 2010-2011
Grades K through 8

I/We _______________________________________ the undersigned agree to to pay the tuition amount of $4550 less the amount of subsidy of up to $1650 we have requested from ______________________, our parish.  It is understood in the event full subsidy is not received from our parish, I/We will be responsible for the difference in tuition due for the total of $4550.

SELECT ONE OPTION (Options 1 and 2 are the only plans provided.)
No student will be admitted into class unless one of the two payment options has been exercised.  Tuition must be paid in full or the family must be actively enrolled in the monthly FACTS tuition payment plan before the first day of class.

_______OPTION 2 - FACTS 10 MONTH PAYMENT PLAN (June – March)
                                   
Paperwork is due to FACTS in May for payments to begin in June.
                                    If FACTS forms are received late, the 10 month plan will change to a nine
                                    month plan or less depending upon the FACTS enrollment date.  All tuition
                                    payments made through FACTS must be completed by March.  A $41
                                    administrative fee will be included for any family choosing this option.

WITHDRAWAL POLICY: Regardless of which option you choose, if your child/ren should withdraw before the start of the school year, only a portion of tuition, not to exceed more than 80% of the total amount of tuition may be refunded subject to Pastor and finance committee approval. If your child/ren should withdraw from the school by November 30th no more than 50% of the total amount will be subject to a refund. After November 30th there will be no refunds on tuition for withdrawal of your child/ren.  In the event of an early departure, any outstanding FACTS monies will be due and payable.  The $200.00 registration fee and/or any late fees are non-refundable. 

REGISTRATIONS will not be accepted from any families with outstanding balances of tuition, before/aftercare and/or lunch.  All accounts must be paid in full and /or up-to-date with approved existing payment plans in order to be eligible for re-registration.  

* If you are registering a student into the Pre-Kindergarten program, please contact the school office to obtain the School Tuition Contract for Pre-K.
